Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs, ANPs & APNPs to join our workforce with opportunities within Out Of Hours. We employ salaried and offer permanent contracts.
Salary: 64,876.00 - 66,711.00 per annum (pro rata) plus unsociable enhancements.
Department: Out of Hours. 24/7 service
Location: Various locations within Merseyside, Sefton & Halton; including Wavertree Head Office.
Shifts: Full time/part time positions available - Weekdays after 6pm and Weekends. Remote working available.
Main duties of the job:
- Assess, diagnose, plan, implement and evaluate treatment interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national local protocols and within scope of practice.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
What do you need:
- You will be an experienced ACP or ANP who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education and research.
- Full NMC Registration with no restrictions.
- MSc in Advanced Clinical Practice is desirable; candidates who are working towards this qualification or who have relevant experience are encouraged to apply.
- Non-Medical Prescriber V300.
- Clinical Examination and Diagnostics Level 7.
- You will have experience of working in the acute sector, urgent/emergency services environment, or out of hours services at a senior level.
