You will be registered with the GPhC or about to complete your foundation training in order to register as a Clinical Pharmacist with the GPhC. You may already be an independent prescriber or working towards this qualification. The role involves proactively supporting patients with complex polypharmacy, particularly older adults and those with multiple long-term conditions. You will have opportunities to develop clinical interests, enhance prescribing skills, and improve patient outcomes.Working across a small number of practices will allow you to build strong relationships within primary care teams. Responsibilities include managing repeat prescription authorisation, acute requests, transfer of care processes, and promoting safer prescribing systems. Ongoing professional development is supported through a clinical network, including up to 28 study days over the first 18 months.
