An exciting opportunity has arisen for an enthusiastic and motivated Clinical Pharmacist to join our friendly and supportive general practice team, working across Marisco Medical Practice and Liquorpond Surgery. This is an excellent opportunity to play a key role in delivering high-quality, patient-centred care while helping to optimise medicines use and improve health outcomes across our two practices. You will work closely with Pharmacists, GPs, nurses, pharmacy technicians, and the wider multidisciplinary team within the practice, across the PCN to provide expert clinical pharmacy services.
Main Responsibilities
- Undertake structured medication reviews and optimise medicines for patients with long-term conditions.
- Provide specialist medication advice to clinicians and patients.
- Support safe, cost-effective prescribing in line with national and local guidance.
- Manage medicines-related queries from patients and healthcare professionals.
- Monitor high-risk medicines and undertake clinical audits where appropriate.
- Manage medication alerts and medication shortages.
- Support prescribing quality improvement initiatives and medicines optimisation projects.
- Participate in the management of repeat prescribing systems.
- Work collaboratively with community pharmacies, secondary care, and other healthcare providers to ensure seamless patient care.
- Contribute to the delivery of enhanced services and practice development.

Job Responsibilities
- Undertake medicines reconciliation following hospital discharge or other transfers of care and update patient records accordingly.
- Work collaboratively with patients, carers and community pharmacies to ensure continuity of medicines following discharge.
- Review and develop repeat prescribing protocols and Standard Operating Procedures.
- Provide prescribing advice to GPs and clinical staff to improve prescribing safety, quality and cost-effectiveness.
- Implement prescribing changes in response to MHRA alerts, medicine shortages and national guidance.
- Maintain accurate clinical records using agreed templates and coding standards.
- Lead and participate in prescribing audits and quality improvement initiatives.
- Produce evidence-based prescribing guidance and educational resources.
- Promote the safe storage, handling and use of medicines and recommend improvements where required.
- Respond professionally to medication-related queries from patients and healthcare professionals.
