Join LIVES and play a pivotal role in transforming urgent and emergency care across Lincolnshire. As a Community Emergency Medicine ACP, you will work autonomously and as part of a multidisciplinary team, delivering advanced assessment, diagnosis, treatment and clinical decision-making in patients' homes and community settings. This is a dynamic frontline role focused on providing the right care, in the right place, at the right time. You will manage a wide range of undifferentiated presentations, support virtual wards, utilise community pathways and help patients avoid unnecessary hospital attendance or admission wherever it is safe to do so.

Main duties of the job:
- The Community Emergency Medicine Advanced Clinical Practitioner (ACP) delivers advanced assessment, diagnosis, treatment and clinical decision-making for patients in community and pre-hospital settings across Lincolnshire.
- Working autonomously and within a multidisciplinary team, the ACP manages a wide range of undifferentiated presentations, supports virtual wards and utilises community pathways to provide safe, effective care closer to home.
- The role focuses on preventing avoidable hospital attendance and admission through holistic assessment of patients' medical, functional and social needs.
- The postholder will identify risks, support early intervention and safeguarding, make referrals to appropriate services, and empower patients and carers to make informed decisions about their care.
- The ACP will operate as part of a mobile clinical team, responding to incidents across the county, undertaking dynamic risk assessments and maintaining accurate clinical records.
- They will work collaboratively with urgent and emergency care partners, primary care, social care and specialist services, while contributing to clinical governance, service improvement, education and mentorship.
- As a member of LIVES, the postholder will also support the charity's mission through community engagement, awareness raising and fundraising activities.
