This full-time role (37.5 hours, Mon to Fri with on-call commitment) is ideal for pharmacists from hospital or community settings seeking to develop specialist mental health expertise. Main duties of the job:
- As a Specialist Clinical Pharmacist, you will provide a high-quality, patient-centred clinical pharmacy service across the Trust.
- Responsibilities include delivering ward-based clinical pharmacy services, optimising medicines use, supporting safe and timely discharge through patient counselling and education, promoting adherence, and ensuring effective communication with community healthcare providers.
- You will also contribute to wider clinical pharmacy and departmental activities to support the Trust's pharmacy service objectives.
