Overview
As a Social Prescriber in HMP Gartree, you help people connect to activities and services to improve health and wellbeing within a prison setting. You will build relationships with patients, healthcare staff, and external providers to promote social prescribing and accessible opportunities. The role supports ongoing professional development and works within a supportive, mission-driven healthcare team. Responsibilities
- Build professional relationships with patients by listening to what matters to them
- Connect patients to prison-based activities and services to improve health and wellbeing
- Motivate and support patients to access opportunities for healthier lives
- Liaise with healthcare staff, prison departments, charities, and third-sector providers to promote social prescribing
- Ensure social prescription opportunities are available across sites
- Engage in ongoing professional development and cross-functional collaboration
Key requirements
- Experience delivering information, advice and guidance in health or care settings
- Experience working in a prison environment or with patients with complex needs
- Good IT skills (Microsoft Office) and SystmOne
- Proactive problem-solving and strong organizational skills
