Seacroft Primary Care Network (PCN) is looking for a highly motivated and enthusiastic Prescribing Clinical Pharmacist to join the team. The PCN pharmacy team currently consists of a Lead Pharmacist, 3 other pharmacists, and a Pharmacy Technician. The team is well integrated into the PCN and their work is valued by all member practices. All PCN roles are supported by the Clinical Director.
You will be working across Seacroft PCN, which consists of 4 well-led practices that deliver excellent care to a patient population of 35,000. The practices are Foundry Lane, Park Edge, Oakwood Lane, and Windmill located in LS8 and LS14. This role will be a hybrid of remote working and being in practice. It is an exciting opportunity to use and develop your clinical knowledge and skills to contribute to medicines optimisation and improve the lives of patients across the PCN.
Applications from all pharmacy sectors are welcome; however, experience and knowledge of working in primary care would be desirable. Being a prescriber is essential. All PCN pharmacy staff are required to enrol on (or be exempted from) the 18-month CPPE Primary Care Pharmacy Education Pathway. If this is not completed, you will be allocated a clinical supervisor in the PCN to complete the training.
Main duties of the job:
- The main aim is to improve outcomes for patients from their medicines and reduce the risk of harm.
- Improve medicines safety and management processes across the PCN.
- Carry out structured medication reviews (SMRs) to optimise medication for complex patients, particularly the frail, polypharmacy, and care home residents.
- Home visits for housebound patients may be required.
- Join established MDTs locally to deliver proactive, personalised, and coordinated health and social care.
- Work collaboratively with the PCN Clinical Director and participate in the PCN MDT.
Job responsibilities:
- Support the South & East Leeds General Practice Group Practices with all aspects of medicines optimisation.
- Work within clinical competencies as part of a multi-disciplinary team.
- Conduct structured medication reviews with direct patient contact.
- Manage long-term conditions and minor illnesses.
- Contribute to repeat prescription authorisations and reauthorisation.
- Address public health and social care needs of patients.
- Contribute to achievement of QOF and quality improvement schemes.
- Undertake clinical audits.
Key duties and responsibilities:
- Patient facing Clinical Medication Review.
- Medicines quality improvement through clinical audits.
- Implement changes to medicines based on MHRA alerts.
- Manage common/minor/self-limiting ailments.
- Provide patient-facing clinics and telephone support for medicine-related queries.
- Reconcile medicines at change of care settings.
- Provide drug monitoring and ensure robust systems are in place.
- Ensure appropriate signposting of patients to healthcare professionals.
- Contribute to service development and education/training for the primary healthcare team.
- Work with general practice teams to ensure compliance with CQC standards.
- Support public health campaigns.
- Engage with pharmacy colleagues across Leeds for peer support.
Professional development:

Qualifications:
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Holds or working towards an independent prescribing qualification.
Other requirements:
- Meets DBS reference standards and has a clear criminal record.
- Access to own transport and ability to travel across the locality.
