Job summaryThe Leeds Diabetes Neighbourhood Health Programme is a new integrated model of care, bringing together primary care, community services and specialist teams to improve outcomes for people living with diabetes and multiple long-term conditions. As part of this innovative programme, we are looking to recruit a dynamic and motivated Highly Specialist Clinical Pharmacist for Diabetes on a 0.4 WTE basis (15 hours per week). The role is based within Leeds Community Healthcare and will primarily support the programme across the Beeston, Middleton and Hunslet areas of south Leeds, working closely with GP practices, community teams and specialist diabetes services. The role offers an opportunity to work across organisational boundaries and care settings, from primary care and neighbourhood multidisciplinary teams to citywide specialist diabetes services. You will work alongside the Consultant Pharmacist for Diabetes, senior clinical leaders and partner organisations to shape and deliver innovative approaches to diabetes care. If you are passionate about diabetes care, collaborative working and driving innovation, we would love to hear from you.Main duties of the jobYou will work as part of the Tier 3 Specialist Multi-Disciplinary Team, providing expert clinical input and leadership for diabetes medicines optimisation across neighbourhood and community services. The role will involve supporting patients with complex diabetes and multiple long-term conditions, developing and implementing medicines pathways, and promoting safe, evidence-based prescribing practice. As the specialist pharmacy lead within the programme, you will provide expert advice and education to healthcare professionals across primary care, including community pharmacists, practice pharmacists and PCN teams. You will act as a key link between clinical practice and service development, helping to design and evaluate innovative approaches to diabetes care. The successful candidate will have substantial experience in diabetes management, advanced clinical pharmacy practice, and leading service improvement. You will be confident working across organisational boundaries, influencing practice, and supporting multidisciplinary teams to deliver excellent patient-centred care. The post offers a flexible hybrid working approach, with time spent in neighbourhood clinical settings, GP practices, community venues and home working, depending on service need.
