Overview
In this Band 6 Practitioner role within LD CRIST, you will deliver specialist community support to people with learning disabilities who are at risk of inpatient admission or placement breakdown. You work in a multidisciplinary team to provide assessments, planning, and risk management, aiming to keep individuals safely in the community. The role offers flexible working arrangements and involves engaging with carers, providers, and health and social care staff to meet complex needs. You will contribute to reducing unnecessary admissions through targeted pathways and support.

Responsibilities
- Triage new referrals from multiple sources
- Conduct clinical assessments and oversee care planning for community patients at risk
- Provide training and advisory support to carers, providers, and staff on Positive Behavioural Support needs
- Offer specialist advice to enable people to remain in the community and prevent hospital admissions
- Monitor pathways (Mental Health Pathway and Positive Behavioural Support Pathway) and coordinate handover to LD Community teams when appropriate
Key requirements
- Registered professional with experience working with people with learning disabilities
- Full UK driving licence and vehicle insured for business use
- Experience in clinical assessment and risk management within a community setting
- Ability to work within a multidisciplinary team
- Strong communication and interpersonal skills
- Collaborative team player
