Overview
In this leadership role you will co-develop and drive a defined Integrated Care Community to deliver population health improvements and reduce inequalities. You’ll unite NHS teams, local councils, and community partners to create joined-up, prevention-focused care at the neighbourhood level. You’ll lead cross-organisation collaboration, data-informed decision making, and service modernization to embed a sustainable, community-led model. This is a high-impact opportunity to shape care delivery within the NHS Ten Year plan and NNHIP.
Responsibilities
- Champion ICC development aligning with national priorities for population health, prevention, self-care, and the National Health Framework
- Build robust cross-ICC collaboration and infrastructure across Morecambe Bay and wider system
- Collaborate with specialist services to create seamless, integrated pathways for local citizens
- Deputise for senior colleagues and provide clear, influential progress reports
- Lead service reviews and support skill-mix planning, recruitment, and new ways of working
- Provide strong leadership and line management for the case management team (Nurses, AHPs, Advanced Practitioners, Administrators)
- Collaborate with the named ICC Clinical Lead and coordinate modernization of services
- Identify innovative solutions and evaluate short cycles of change leveraging local assets
- Support co-production of projects/events with citizens and community organisations
Key requirements
- Educated to degree level or equivalent in a relevant area
- Managerial or clinical leadership within a health or social care setting
- Project management of significant and multiple projects
- Improvement measurement systems and understanding of the role of measurement in performance improvement
- Able to demonstrate change management experience
- Communicates effectively with diverse partners
- Excellent interpersonal skills across a wide range of stakeholders
- Well organized with ability to meet objectives within deadlines
- Excellent analytical skills
- High level of Microsoft Office and data analysis skills
- Ability to use data to inform decisions
