You will work with an MDT to undertake medication reviews, support long‑term condition management and provide prescribing guidance. You will contribute to QOF achievement and PCN DES delivery while advancing medicines optimisation and reducing health inequalities. This role sits at the heart of improving patient outcomes in a network that serves around 35,000 patients.
