b Overview /b p In this role you will split your week between acting as the responsible pharmacist in our community pharmacies and delivering frailty services within CHCP. You will work with a multidisciplinary team to deliver comprehensive geriatric care across community settings and hospital-at-home initiatives. You will contribute to NHS pharmacy services, including vaccinations and medicines management, while developing in a supportive, values-driven environment.
