Overview
You will join the REACT and Hospital at Home teams to deliver medical reviews in patients’ homes, planning complete care episodes and coordinating diagnostics and interventions with the multidisciplinary team. The role offers exposure to acute medicine and rehabilitation/geriatric care, with consultant support and educational opportunities. It suits those aiming for GP or geriatric training and values patient-centered, community-based care. You will help optimise outcomes and resource use while developing your skills in a dynamic NHS setting.

Responsibilities
- Provide medical reviews for patients on REACT and Hospital at Home caseloads in their home environment
- Plan and manage complete episodes of care, including diagnostics and interventions
- Initiate and adjust therapeutic plans in collaboration with the multidisciplinary team
- Delegate tasks and refer as appropriate to optimise health outcomes and resource use
- Participate in daily handovers and MDT discussions with senior medical cover available
- Contribute to quality of care and professional development within the team
Key requirements
- Full GMC registration
- MBBS or equivalent
- MRCP Part 1
- Some experience at CMT/VTS level in acute geriatric and general medicine
- NHS experience
- Evidence of multidisciplinary team working
- Effective communication skills
- Flexibility to respond to changing service needs
- Valid driving licence to operate in the community
