Overview

In this role you will coordinate complex patient care within a busy primary care setting, working with clinicians and the wider care team to ensure timely, safe pathways. You will guide and support other care coordinators, using professional judgement to resolve issues and escalate when needed. The role focuses on improving processes, patient experience and service efficiency while maintaining high standards of care. You will contribute to service development and work independently to manage competing priorities. Responsibilities

  • Coordinate patient care for complex cases and ensure timely follow-up and escalation
  • Collaborate with GPs, nurses, clinicians and multidisciplinary teams to support effective patient pathways
  • Provide guidance to Care Coordinators and maintain consistent standards
  • Handle complex or sensitive queries using professional judgement and escalation where appropriate
  • Prioritise workload and ensure actions are completed within timescales
  • Identify opportunities to improve processes, patient experience and efficiency
  • Act as a senior point of reference for colleagues and own delegated responsibilities
  • Contribute to team meetings, service development and practice improvement initiatives

Key requirements

  • Significant experience in a healthcare coordination or similar role
  • Experience of coordinating patient care and managing referrals or signposting
  • Experience of working with clinical and non-clinical colleagues
  • Ability to work independently and manage competing priorities under pressure
  • Experience using clinical/patient management systems and Microsoft Office
  • Demonstrable ability to guide or mentor colleagues
  • Discretion and ability to handle confidential information
  • Excellent organizational and time-management skills
  • Strong communication skills (written and verbal)
  • Professional, approachable and compassionate manner
