Overview
In this role you will lead the day-to-day delivery of hospital-at-home services, ensuring safe, high-quality care and KPI achievement. You will supervise a team of Service Coordinators, coordinate clinical capacity, and work with clinical and service partners to ensure timely support for patients. The position operates on a 7-day rota, requires collaboration with multiple teams, and focuses on improving community-based care while reducing hospital admissions. This is an opportunity to shape at-scale, integrated primary care with a proactive, collaborative leadership style.

Responsibilities
- Manage operational delivery of hospital-at-home services to ensure safe, high-quality patient care
- Oversee staff rotas, visiting lists, service capacity, and clinical workload with appropriate skill mix
- Provide data to demonstrate service performance and drive improvements
- Lead and support the Service Coordinator team with supervision and development
- Recruitment, induction, and ongoing staff development
- Represent services in governance and delivery meetings; liaise with GPNs data and performance teams
- Oversee incident reporting, investigations, and escalation of issues as needed
- Ensure adherence to national standards and local governance; contribute to policies and procedures
- Participate in on-call rota and deputise for Operations Manager when required
- Support and implement new projects and initiatives to improve service delivery
- Prepare and manage HR processes including pay, payroll sign-off, and invoicing
- Conduct monthly audits to ensure regulatory compliance and action planning
- Deal with escalated patient/family queries and complaints
Key requirements
- Proven management experience including service delivery and staff management
- Line management of staff and ability to manage sickness, leave, and rotas
- Experience delivering supervision and appraisals with development plans
- Successful cross-professional collaboration and change management
- Knowledge of NHS community services and clinical governance
- Ability to work across a seven-day week and participate in on-call rota
- Eligibility to work in the UK and DBS clearance
