As a pharmacist working in our team, you will deliver excellent pharmaceutical care and medicines optimisation to achieve the best outcome for our patients.Main duties of the jobYour responsibilities will include:Providing expert clinical pharmacy input to patients in our front door and admissions areas.Utilising your independent prescribing qualification to support medicines reconciliation, optimisation, and discharge planning.Providing wider clinical pharmacy services within the hospital.Collaborating closely with medical, nursing, and allied health professionals to ensure seamless patient care.Contributing to service development and quality improvement initiatives.Supporting the development of current and future pharmacy professionals, as well as other healthcare professionals, through clinical mentorship.We recognise that our profession's future leadership will come from pharmacist like you with drive and ability.
