Overview
As Clinical Pharmacist Lead, you will guide the PCN pharmacy team to deliver high-quality, patient-centred clinical care. You will shape clinical decision-making, safety monitoring, and medicines optimisation across complex cases. You’ll translate national and local priorities into practice and collaborate with partners to improve pathways and outcomes. This is a mission-driven role in a supportive, development-focused PCN with a strong emphasis on continuous improvement.
Responsibilities
- Lead and mentor the pharmacy team, support clinical decision-making and assessment of competencies
- Provide operational leadership for medicines reconciliation, prescribing queries, electronic repeat dispensing and shared-care
- Promote medication safety, respond to medicines alerts and monitor high-risk therapies
- Translate national and local priorities into effective practice
- Deliver structured medication reviews and medicines optimisation for patients, including those with long-term conditions or complex needs
- Run patient-facing clinics (in person, telephone, or domiciliary visits)
- Lead audits, searches, and quality-improvement projects using data to report progress
- Collaborate with GPs, nurses, practice managers, and other partners to improve pathways and outcomes
- Support PCN DES and Neighbourhood priorities, CQC readiness, and health equity across practices
- Oversee key medicines-related programmes
Key requirements
- Masters in Pharmacy and GPhC registration
- CPPE completion
- IP qualification (essential)
- Significant experience in a clinical pharmacist role in primary care
- Experience supervising pharmacy staff
- Knowledge of PCN DES, NHS Long Term Plan and prescribing guidance (NICE/NHSE)
- In-depth knowledge of medicine optimisation, polypharmacy and therapeutics
- Experience in service development or quality improvement initiatives
- Understanding of clinical governance, audit, risk management and incident reporting
- UK professional registration (required)
- Desirable leadership qualification or experience
- Desirable experience with achieving PCN DES, QOF or IIF targets
- Desirable experience with Systm0ne clinical records
