Job summaryThe post holder will be responsible and provide operational management and clinical leadership for the community cardiac and respiratory clinical team to deliver optimal care in line with the Care Closer to Home model and emerging Neighbourhood Health.Use highly developed specialist knowledge to assess, plan, implement and evaluate individualised care for patients with heart failure and respiratory conditions.
