b Overview /b p In this role you will lead and support crisis assessments and care planning for children and young people, working with families, hospital staff and community teams to enable safe discharge and timely follow-up. You will collaborate across Mindworks Alliance partners to develop robust crisis plans and ensure continuity of care.
