Advanced Clinical Practitioner in Halifax

Caritas Group Practice is seeking an experienced and motivated Advanced Clinical Practitioner to join our friendly, forward-thinking practice serving approximately 8,000 patients across three sites in North Halifax. This is an exciting opportunity to become part of a highly skilled multidisciplinary team committed to delivering outstanding, patient-centred care. As an Advanced Clinical Practitioner, you will work autonomously within your scope of practice, assessing, diagnosing and managing patients with a wide range of acute and long-term conditions while working collaboratively with GPs and the wider clinical team.

Main duties of the job:

• The successful candidate will provide expert, autonomous clinical care within a busy and supportive general practice, managing patients with a broad range of acute, urgent and long-term conditions.
• Working as an integral member of our multidisciplinary team, you will assess, diagnose, treat and, where appropriate, prescribe independently, ensuring patients receive timely, high-quality care.
• You will play a key role in improving access to primary care by managing your own clinical workload, making evidence-based decisions and referring patients appropriately when required.
• In addition to direct patient care, you will contribute to health promotion, preventative care, clinical governance, quality improvement initiatives and service development.
• Working closely with GPs, Practice Nurses, Clinical Pharmacists, First Contact Practitioners and other healthcare professionals, you will help deliver safe, effective and person-centred care while supporting the ongoing development of our clinical services.

Job responsibilities:

• Assess, examine, investigate, diagnose and manage patients presenting with acute, urgent and long-term conditions.
• Independently prescribe medications in accordance with current legislation and best practice (where qualified).
• Order, interpret and act upon diagnostic investigations including pathology and imaging.
• Formulate appropriate management plans and provide follow-up where required.
• Refer patients appropriately to secondary care and community services.
• Provide clinical advice, health education and health promotion to patients and carers.
• Undertake home visits where appropriate.
• Recognise and manage clinical risk, safeguarding concerns and medical emergencies.
• Maintain accurate, timely and contemporaneous clinical records using SystmOne.
• Work within national and local clinical guidelines and practice protocols.

Service Delivery:

• Manage a defined clinical workload, including same-day, urgent and routine appointments.
• Support effective patient flow and improved access to services.
• Work collaboratively with GPs, Practice Nurses, Healthcare Assistants, Clinical Pharmacists, First Contact Physiotherapists, Mental Health Practitioners, Social Prescribers and other members of the multidisciplinary team.
• Participate in multidisciplinary team meetings where appropriate.
• Contribute to service redesign and the implementation of new models of care.

Clinical Governance:

• Maintain high standards of clinical practice and professional conduct.
• Participate in clinical audit, quality improvement and service evaluation.
• Contribute to significant event reviews and clinical governance activities.
• Support the implementation of national guidance and local policies.
• Ensure compliance with infection prevention and control procedures.
• Maintain patient confidentiality and comply with GDPR and information governance requirements.

Leadership and Professional Responsibilities:

• Demonstrate leadership within the clinical team.
• Act as a role model for evidence-based practice.
• Support and mentor junior colleagues, students and newly appointed staff where appropriate.
• Participate in appraisal, mandatory training and continuing professional development.
• Maintain registration with the appropriate professional body and comply with revalidation requirements.
• Keep up to date with developments in advanced clinical practice and primary care.

Communication:

• Communicate effectively with patients, relatives, carers and colleagues.
• Work collaboratively with external healthcare professionals and partner organisations.
• Manage sensitive conversations with professionalism, empathy and confidentiality.

Health and Safety:

• Comply with all practice Health and Safety policies.
• Report incidents, accidents and near misses in accordance with practice procedures.
• Maintain a safe working environment for patients, visitors and colleagues.
