The NHS Primary Care Network (CCOB PCN) is seeking a Clinical Pharmacist with an Independent Prescriber qualification to join four Kingston practices: The Churchill Medical Centre, The Orchard Practice, Canbury Medical Centre and Berrylands Surgery.
You will conduct medication reviews, support chronic disease management, and lead medicines quality improvements across the network while collaborating with local pharmacists and prescribing clerks.
