Brockwood Medical Practice is looking to recruit an enthusiastic and motivated Clinical Pharmacist to join our expanding multidisciplinary team. This is an exciting opportunity for a pharmacist who is passionate about delivering high-quality patient care within general practice and who wishes to continue developing their clinical knowledge and prescribing skills.

Main duties of the job:- Undertaking structured medication reviews.
- Supporting patients with long-term conditions.
- Optimising medicines and reducing prescribing risks.
- Managing medication queries.
- Supporting care home and frailty patients.
- Undertaking clinical audits and quality improvement projects.
- Supporting QOF, enhanced services and prescribing optimisation.
- Working collaboratively with GPs, nurses and the wider primary care team.
- Providing prescribing advice and education to colleagues.

Job responsibilities:- Undertake structured medication reviews for patients with complex medication regimens.
- Optimise medicines for patients with long-term conditions.
- Conduct medication reviews following hospital discharge.
- Manage medicines reconciliation following discharge from hospital or outpatient appointments.
- Provide prescribing advice to GPs and other clinicians.
- Support safe prescribing for patients receiving high-risk medicines.
- Undertake medication monitoring in accordance with national and local guidance.
- Review repeat prescribing requests.
- Respond to medicines-related patient queries.
- Manage prescribing for care home residents where appropriate.
- Participate in multidisciplinary team meetings.
- Provide patient education regarding medicines and treatment options.
- Support deprescribing where clinically appropriate.
- Monitor prescribing safety alerts and implement necessary actions.
- Promote evidence-based prescribing.
- Undertake prescribing audits.
- Support QOF achievement.
- Support Medicines Optimisation initiatives.
- Improve prescribing quality indicators.
- Reduce medicines waste.
- Monitor prescribing budgets where appropriate.
- Implement MHRA safety alerts.
- Support implementation of NICE guidance.
- Participate in clinical audit.

- Support achievement of national and local contractual requirements.
- Assist with CQC preparation.
- Identify prescribing risks.

- Support antimicrobial stewardship.
The Clinical Pharmacist will:
- Support trainee Clinical Pharmacists.
- Support Community Pharmacist placements.
- Contribute to multidisciplinary education.
- Deliver medicines education sessions to practice staff.
- Participate in practice educational meetings.
- Maintain their own continuing professional development.
Qualifications:
- Master's degree in Pharmacy (MPharm) or equivalent.
- Registered with the General Pharmaceutical Council (GPhC).
- Clinical experience in primary care, community pharmacy or hospital pharmacy.
- Evidence of continuing professional development.
Desirable:
- Independent Prescriber qualification.
- Primary Care experience.
- Completion (or working towards) the Primary Care Pharmacy Education Pathway.
- Experience using SystmOne.
- Experience of clinical audit.
- Experience of quality improvement.
- Experience of QOF and Enhanced Services.
- Experience of supervising or mentoring learners.
