Overview
In this Advanced Clinical Practitioner role, you will deliver specialist palliative and end-of-life care alongside the Palliative Care Consultant, across community, hospital and care home settings. You will work with senior leaders and partner organisations to shape services that reflect coproduction and a compassionate ethos, while ensuring care is safe, evidence-based and aligned with national standards. Your involvement spans clinical practice, leadership, education and research to support growth and high-quality care. Responsibilities
- Provide specialist care to complex palliative patients in collaboration with the Palliative Care Consultant
- Support team management and service development to reflect quality standards and coproduction
- Contribute to service growth and alignment with local/national guidelines
- Demonstrate advanced clinical skills across four pillars: clinical practice, leadership, education, and research
- Work across multiple settings (community, hospital, care homes) as appropriate
- Engage in clinical governance, risk management and quality improvement initiatives
- Deliver education and present to teams and partners
- Act as a leader and educator for service areas
Key requirements
- Registered with NMC/HCPC
- Masters degree or equivalent in Advanced Clinical Practice
- Non-medical prescribing qualification
- Teaching qualification
- Independent prescriber
- Extensive clinical experience in community/outpatient settings
- Experience leading a team and conducting audits
- Ability to access sites around North East Lincolnshire
- Excellent verbal and written communication
- Ability to build rapport and trust with patients and multidisciplinary teams
- Leadership and empowerment of multidisciplinary teams
