Overview

In this role you join the Waverley Neighbourhood Mental Health team to lead and support high-quality, person-centred care for people with frailty, dementia and related mental health needs. You will work within a multidisciplinary team to develop pathways, supervise staff, and drive safe, effective transitions of care. The role combines clinical leadership with service development to reduce unnecessary assessments and improve outcomes. Responsibilities

  • Support the service manager in daily operations of the Frailty, Later Life and Dementia service line
  • Ensure safe, effective, high-quality service delivery within the community neighbourhood team
  • Provide leadership and supervision for staff on complex cases
  • Collaborate as part of the community neighbourhood clinical leadership team to meet patient needs
  • Conduct patient assessments, care planning, and maintain records; provide highly specialist advice
  • Liaise with external agencies to plan care or education programmes
  • Develop and maintain effective clinical pathways within the service line and wider team
  • Facilitate safe transitions in and out of the service line

Key requirements

  • Significant experience at band 6 or higher
  • Degree with post-registration/postgraduate specialist training; CPD or equivalent; PSPA/mentorship qualification (if applicable)
  • NMC, HCPC, or SWE registration
  • Valid driving licence and access to a car/motorcycle for work travel
  • UK registration and DBS check required
  • Desirable: experience of leading clinical practice
