Job summaryA new opportunity has arisen for an experienced Clinical Pharmacist to join Glossop PCN. You will work as part of a multidisciplinary team responsible for providing a comprehensive Medicine service, providing appropriate medicine and prescribing information and advice.This role is pivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice. The pharmacist will provide direct, accessible and timely medicines expertise; including face to face contact with patients and embracing new ways of working including telephone and video consultations.Main duties of the jobThe role involves providing expert pharmaceutical care through patient-centred medication reviews, medicines optimisation and safe prescribing support. The successful candidate will manage patients with long-term conditions, address medication-related concerns, promote self-care and shared decision-making, and support repeat prescribing.
