Overview
In this role you will lead expert clinical screening and decision-making to route urgent and specialist referrals within a county-wide mental health pathway for children and young people. You work closely with cross-agency partners to ensure timely access to appropriate services, shaping care pathways and improving outcomes. The post demands advanced clinical reasoning, risk assessment and safeguarding know-how in a fast-paced, collaborative environment. Responsibilities
- Provide expert telephone screening and triage for urgent and specialist referrals
- Assess clinical need, risk, safeguarding concerns and urgency using advanced clinical judgement
- Make autonomous decisions on referral prioritisation and allocation
- Allocate referrals to appropriate Surrey pathways (e.g., Urgent Care, CEDS, Mindworks Hubs, SBN, EIIP, CLD, Transfer of Care, Voluntary Sector Alliance Services)
- Demonstrate knowledge of Neurodevelopmental pathways and medication referral processes
- Develop safety plans and escalate urgent referrals per local guidance
- Support Hub Screening Clinicians with routine referrals when demand allows
- Provide clinical advice and consultation to colleagues and partner agencies
- Promote consistent decision-making through supervision and SOP adherence
- Support induction, supervision, and development of junior staff and students
- Act as a clinical role model, promoting evidence-based practice and Trust values
- Communicate effectively with children, young people, families and professionals
- Provide timely clinical advice to referrers, GPs, schools, social care, and partner organisations
- Build relationships across the Mindworks Alliance and represent the Access and Advice Team at meetings
- Present complex cases in team meetings and formulation discussions
- Support safe and timely patient flow by screening and allocating referrals promptly
- Collaborate with Administration Team to manage referrals, inquiries, and inbox
- Provide timely clinical decisions to support referral processing and allocation
- Document all clinical decisions in the electronic patient record
- Escalate operational or capacity concerns to the Service Manager
- Support service performance by maintaining records and contributing to KPI reporting
Key requirements
- Registered Mental Health Professional (RMN, Social Worker, Occupational Therapist or equivalent)
- Extensive experience as a band 6 nurse/occupational therapist or social worker
- Experience of telephone or remote clinical triage, assessment and clinical decision-making in a mental health setting
- Experience of working in multi-agency systems (schools, social care, GPs, acute services and voluntary sector)
