Overview

In this role you support the Single Point of Contact to streamline access to community services and prevent hospital admissions. You will communicate with clinicians, patients, and carers to gather and share up-to-date information, coordinate action plans, and liaise with multiple therapies and agencies. The role sits within a team that connects patients to timely, multidisciplinary care, contributing to compassionate, patient-centered services. You will work in a central hub that supports cross-functional collaboration and efficient referrals.

Responsibilities

  • Cover the Single Point of Contact referral service, handling referrals from clinicians and patients for community services
  • Accurately complete referral forms with checks and up-to-date information for health professionals prior to patient assessment
  • Liaise with a range of health professionals to identify and coordinate appropriate action plans for patients
  • Negotiate with multiple therapies/agencies to ensure timely multi-disciplinary assessments
  • Act as information point of contact for professional callers and record accurate telephone messages
  • Update patient records electronically (Rio) and maintain databases/spreadsheets
  • Provide full secretarial and administrative support as required
  • Set up and maintain comprehensive filing and patient record systems for easy retrieval
  • Manage incoming post distribution and arrange meetings, training, and equipment as needed
  • Maintain stocks of day-to-day stationery items

Key requirements

  • GCSE or O level or equivalent including Maths and English (grade C or above)
  • NVQ level 3 in Business and Administration or equivalent
  • IT experience with Microsoft Office
  • RSA III or equivalent audio typing
  • Working knowledge of clerical/office procedures
  • Confidentiality and data protection awareness
  • Ability to communicate with professionals and managers at all levels (verbal and written)
