As a Clinical Pharmacist, you will be part of our valued Hospital at Home team working across our BSW hubs, helping to optimise medicines, improve patient outcomes, and support personalised care for some of the most complex patients in our communities. You will provide expert pharmaceutical advice, contribute to Hospital at Home MDT discussions, and support clinical decision-making.
You will lead on medicines optimisation for older people living with frailty, ensuring prescribing is evidence-based, cost-effective and tailored to individual needs. This role offers the opportunity to influence service development, contribute to clinical governance and safety initiatives, develop guidance and protocols, and support innovative models of care that reduce hospital admissions and improve patient experience.
As the Clinical Pharmacist for Hospital at Home, you will lead, deliver and continuously develop a safe, effective and high-quality clinical pharmacy and medicines optimisation service for patients receiving acute care at home. Working as a key member of the multidisciplinary team, you will champion safe, evidence-based and cost-effective prescribing, ensuring medicines are optimised to improve patient outcomes and experience.
The Ideal Candidate:
- Masters degree in Pharmacy or equivalent, registered with the GPhC as a pharmacist.
- Clinical Pharmacy Diploma (2 years) or equivalent experience, with evidence of continuing professional development.
- At least 2 years experience working in a clinical pharmacy role, including experience working within a multidisciplinary team.
- Good knowledge of medicines legislation and national standards.
- Excellent clinical knowledge and effective written and verbal communication skills in English.
