Overview
In this role, you will join a Clinical Pharmacy team that supports three GP practices to optimise medicines and improve patient outcomes. You will work with GPs and practice staff to deliver patient-facing reviews, prescribing optimization, and service development, contributing to high-quality primary care. You’ll lead on safe, effective, and cost-conscious medicines use while advancing governance and improvement activities. This is a patient-centered, research-connected practice with opportunities to shape local services and NHS-focused aims.

Responsibilities
- Undertake clinical medication reviews and structured medication reviews (including care home residents and long-term conditions)
- Deliver patient-facing consultations (face-to-face, phone, or digital) with medicines advice and referrals management
- Provide safety-netting, lifestyle advice, and appropriate signposting
- Collaborate within multidisciplinary teams to optimise patient care
- Lead medicines optimisation for safe, effective, and cost-efficient prescribing
- Manage repeat prescribing and medicines reconciliation for safe transitions of care
- Identify and manage prescribing risks and reduce avoidable hospital admissions
- Lead and support clinical audits, quality improvement, and service development
- Use prescribing data to improve outcomes and implement local/national guidance
- Support CQC compliance, incident reporting, and delivery of PCN DES, QOF, and public health priorities
- Provide clinical leadership and support for Pharmacy Technicians and practice staff
- Develop and improve prescribing systems and services
- Maintain accurate clinical records and up-to-date clinical knowledge
- Maintain GPhC registration and CPD
- Ensure compliance with governance, safeguarding, confidentiality, health and safety, and mandatory training
Key requirements
- GPhC registration (current)
- Undergraduate degree in Pharmacy
- Willingness to work towards Independent Prescribing Qualification
- Minimum of 2 years as a Pharmacist
- Experience with common acute and long-term conditions in general practice
- Excellent communication and understanding of primary care prescribing and governance
- Ability to identify and manage risk in long-term conditions
