As a Bank Enhanced Clinical Practitioner (ECP), you'll be part of our valued team in the Hospital at Home service based across community bases within Swindon or Wiltshire (North, East, South or West). As an Enhanced Clinical Practitioner, you'll work within the Hospital at Home service, delivering acute-level care in patients' homes. Your role involves preventing hospital admissions and supporting safe discharges. Working with a multidisciplinary team, you'll provide evidence-based care, lead clinical decision-making, and contribute to service development.
The ideal candidate will have:
- Current professional registration with the NMC or HCPC
- Degree-educated with relevant postgraduate learning or equivalent experience in enhanced clinical practice
- Recognised qualification in enhanced/advanced clinical assessment and diagnostic reasoning
- Significant post-registration experience in elderly care, frailty, community services, or a relevant acute specialty
- Strong experience in urgent and/or acute clinical assessment
- Proven ability to independently manage complex caseloads and make autonomous clinical decisions
- Excellent communication skills and strong clinical judgement
- Current full driving license and use of a car during working hours
