Overview
In this role you will deliver specialist clinical pharmacy support to community mental health teams in Torbay and Totnes, collaborating with primary care and mental health services.

Responsibilities
- Develop and provide specialist clinical pharmacy support to Mental Health community teams
- Build connections with local primary care (PCN) services and collaborate with other mental health services as needed
- Conduct medication reviews, counselling, prescribing, and audits
- Engage in physical health initiatives and training activities
- Support inpatient admissions for local patient groups where appropriate
- Key requirements
- Up-to-date knowledge of guidelines and clinical practice in the specialty
- Experience working in a multidisciplinary healthcare team
- Ability to supervise pharmacy technicians and assistants
- Experience with change management and implementing change in complex settings
