Overview
In this Foundation Pharmacist role, you contribute to medicines optimisation across wards as part of the hospital Pharmacy team. You will rotate through ward specialities under supervision, gaining hands-on experience with a multidisciplinary team. Supported by the practice supervisor and Education & Training, you develop clinical skills, including safe, evidence-based prescribing and patient discharge counselling. The role emphasizes formulary adherence and antimicrobial stewardship within the Trust’s guidelines, offering a growth path in hospital pharmacy.
Responsibilities
- Provide clinical pharmacy support across a group of wards during rotations under supervision
- Deliver medicines optimisation to ensure safe, effective, and cost-conscious use of medications
- Counsel patients and carers about discharge medications and liaise with primary care for continuity of care
- Develop skills through rotations including Medicines Information handling and responding to enquiries
- Implement the Trust Medicines Formulary and advise medical staff on guideline adherence
- Support antimicrobial guidelines adherence and safe prescribing within the Trust
- Collaborate with Medicines Management Technicians to facilitate patient understanding and continuity of care
Key requirements
- MPharm degree or equivalent qualification
- GPhC registration as a pharmacist
- Evidence of Continuing Professional Development (CPD)
- IT literacy; ability to prioritise tasks effectively
- Strong written and verbal communication; good interpersonal and influencing skills
- Experience working in a multi-disciplinary healthcare setting
- Desirable: Postgraduate Diploma in Clinical Pharmacy or working towards; member of Royal Pharmaceutical Society
- Desirable: Hospital Pharmacy experience
- UK professional registration (GPhC) current
