Overview
As a Clinical Pharmacist in the EHICH Team, you will lead medicines optimisation for care homes, collaborating with MDTs to optimise medication use and safety. You will support prescribers, Pharmacy Technicians, and care coordinators, delivering medication reviews and expert guidance on complex polypharmacy. The role focuses on frailty, COPD/asthma, and learning disabilities, addressing public health and social care needs. You will contribute to antimicrobial stewardship, quality improvement, and reducing GP workload within the PCN and Darlington practices.

Responsibilities
- Be an independent prescriber or commit to training to become one
- Support colleagues and patients with medication queries and monitoring
- Undertake clinical medication reviews for complex polypharmacy
- Focus on care home residents with multiple comorbidities (frailty, COPD, asthma) and learning disabilities/autism
- Provide specialist medicines expertise addressing public health and social care needs
- Lead audits and action planning to tackle inequalities
- Lead person-centred medicines optimisation and promote antimicrobial stewardship
- Contribute to QOF and enhanced services in General Practice
- Use structured medication reviews to optimise patient outcomes and promote self-care
- Provide pharmaceutical advice for new medicinal services and treatment pathways
- Ensure practice compliance with CQC standards relating to medicines in care homes
- Develop relationships with other pharmacy professionals and broader health/social care systems
- Take a central role in shared care protocols and liaise with specialist/community pharmacists
- Undertake CPPE Primary Care Pharmacy Education Pathway or equivalent
- Provide clinical supervision for Pharmacy Technicians within the PCN EHICH team
- Assist with care home medication requests and covert administration guidance
Key requirements
- Degree in Pharmacy
- Registration with General Pharmaceutical Council
- Completed or willing to complete CPPE pathway or equivalent
- Independent Prescribing qualification (desirable)
- Minimum 2 years experience as a pharmacist (desirable)
- Proven ability to recognise and manage risk
- Ability to work flexibly with changing priorities
