b Overview /b p In this role you will join a cross-trust team to develop and consolidate geriatric medicine and frailty services for both acute and community settings. You will work across Lewisham Greenwich Trust and Oxleas NHS Foundation Trust, splitting duties between Queen Elizabeth Hospital and Memorial Hospital, with a focus on integrating care from hospital to community neighbourhood teams. You will lead, mentor staff and participate in education, research, and governance to improve outcomes for older people. /p b Responsibilities /b ul li Support development of comprehensive geriatric medicine service /li li Develop frailty initiatives within Greenwich integrated neighbourhood teams /li li Enhance interface frailty services for ED and other clinical areas /li li Lead and mentor junior medical staff; ensure training requirements are met /li li Collaborate with ICBs and community partners for integrated care /li li Be an advocate for older people within the organization /li li Lead and participate in MDT workflows and ward rounds /li li Contribute to research and quality improvement /li li Meet targets for outpatient and ED waiting times /li li Provide undergraduate and postgraduate teaching and supervision /li /ul b Key requirements /b ul li MBBS or equivalent /li li MRCP or equivalent /li li GMC registration and licence to practise /li li Specialist Register for Geriatric Medicine or within six months of CCT /li li Broad-based training in acute, general and geriatric medicine /li li Experience in clinical governance and audit /li li Commitment to teaching and training /li li Willingness to take on additional professional responsibilities /li li Good communication and leadership of a multidisciplinary team /li /ul ul li Good communication /li li Leadership /li li Multidisciplinary collaboration /li li Geriatric medicine /li li Frailty assessment and management /li li Clinical governance and audit /li /ul
