b Overview /b p In this role you will lead high-quality care for older adults, conducting specialist cognitive assessments and managing functional mental disorders in outpatient settings. You will head the older adults community MDT at Hartley Hospital, coordinating with diverse clinical teams to assess new referrals, form clinical plans, and optimise quality of life. You’ll drive service development, improve care pathways between primary and secondary services, and provide expert guidance in a consultative, supportive manner. The position sits within a broader transformation programme focused on safe, timely, equitable care and service improvement. This is a clinically impactful role with opportunities to影 /p b Pay / Benefits /b ul li Salary 113,565–150,569 per year /li li 7.5% R R (non-contractual) /li li 1% on-call /li li Flexible working requests considered /li /ul b Responsibilities /b ul li Act as Responsible Clinician for patients in the caseload within the older adults CMHT /li li Provide leadership, supervision and clinical input to the multidisciplinary team /li li Develop and improve the service and care pathways between primary and secondary care /li li Conduct new patient assessments, formulate diagnoses and plan treatments /li li Oversee monitoring and titration of treatments, with follow-up at defined intervals /li li Coordinate with GPs, primary care staff, patients and carers for timely interventions /li li Utilise Rio electronic health record for clinical documentation and reporting /li li Engage in annual appraisals, CPD and GMC registration maintenance /li li Participate in service modernisation and adherence to policies and financial instructions /li /ul b Key requirements /b ul li Full GMC registration /li li Inclusion on or eligibility for Specialist Register in Old Age Psychiatry or within 6 months of CCST/CCT /li li Section 12(2) Approval for Approved Clinician status /li li MbChb, MBBS or equivalent /li li Valid UK driving licence or suitable alternative /li li Excellent clinical knowledge and communication skills /li li Ability to manage clinical complexity and uncertainty /li li Experience with MHA and MCA duties (desirable: DoLs training) /li /ul ul li leadership and influence /li li clear, empathetic communication /li li collaborative teamworking /li li Rio electronic health record system /li li IT systems familiarity /li li clinical audit and service evaluation /li /ul
