Overview
Join the Community Frailty team in Northeast Essex as a consultant (LTFT or full-time) to lead integrated frailty services across acute, community and virtual care settings. You will drive Comprehensive Geriatric Assessment, proactive frailty management and admission avoidance while working closely with multidisciplinary colleagues. The role supports care at patients’ usual residence when clinically appropriate, leveraging digital systems and a collaborative, patient-centred culture. This is a chance to shape services in a large NHS trust focused on timely, high-quality care.

Responsibilities
- Provide consultant-led care with high standards
- Develop and implement care pathways and clinical guidelines
- Maintain effective practices with wards, out-patients and ED
- Foster links with primary care and other departments
- Offer educational, clinical leadership to community and acute teams
- Monitor performance and benchmark service quality
- Oversee junior doctor training and participate in appraisals
- Fulfil the role of community Geriatrician across frailty syndromes
- Collaborate on protocols, guidelines, and clinical audits
- Provide advisory services to other specialties and the mental health trust
Key requirements
- Full GMC registration
- Entry onto Specialist Register or UK SpR within 6 months of CCT
- MRCP or equivalent
- Strong practical clinical skills
- Experience teaching postgraduate and undergraduate staff
- Ability to give expert clinical opinions on acute emergencies, rehabilitation and intermediate care
- Minimum 4 years at SpR level (or equivalent)
- Experience in managing frailty syndromes and admission avoidance
- Experience in community settings and knowledge of community services
- Ability to lead and work in a multidisciplinary team
- Excellent communication and interpersonal skills
- Ability to organise and prioritise work; willing to travel for the post
