Job summaryEAST:MK Primary Care Network has exciting opportunity for a clinical pharmacist to join our practice team, on a full-time basis of 37.5 hours a week. The hours will be worked over 5 days between Monday-Saturday, on a rotation basis across the PCN's 4 surgeries: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice & The Grove Surgery.Our clinical pharmacist will work collaboratively with the multidisciplinary team (MDT) within the PCN, ensuring safe, evidence based & cost-effective prescribing. You will also be responsible for offering support to general practice staff in relation to prescription & medication queries as well as conducting medicines reconciliation on transfer of care.Main duties of the jobWork as part of a multidisciplinary team in a patient-facing roleTake on responsibility for areas of chronic disease management, working proactively with other healthcare professionals across the organisation & PCN areaSupport the delivery of the new Network Contract Directed Enhanced Service (DES)Conduct structured medication reviews to proactively manage patients with complex polypharmacyProvide clinical medicines expertise to both patients & practice teamsImprove prescribing practices across the PCNImprove service & quality through mechanisms such as clinical auditsInterface with community & hospital colleagues to help resolve medication-related issuesAssist with delivery of enhanced health-in-care-homes initiativesAbout usEast:MK PCN covers a total population of around 58,000 patients, across its 4 practices in Milton Keynes: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and the Grove Surgery.Our overarching objective is to deliver the highest standard of care.
