Based at Central Middlesex Hospital, you will deliver a safe, effective clinical pharmacy service to support the enhanced recovery pathway within the Elective Orthopaedic Centre. You will help manage the ward pharmacy service, drive service improvements, and contribute to a pre-op pharmacy service across LNWH. The role involves multidisciplinary collaboration, medication reconciliation, and facilitating rapid discharge while ensuring cost-effective medicines use. This is a clinically focused, fast-paced posting with weekend/on-call duties.
Responsibilities
- Provide safe, effective clinical pharmacy service from admission to discharge per Trust policies
- Perform accurate medication reconciliation to minimise admission-related errors
- Maintain high pharmaceutical input within a multidisciplinary team to optimise medicine use and cost
- Facilitate speedy discharge by organising medications per Dispensing for Discharge policies
- Support same-day discharge pathway implementation with the MDT
- Communicate effectively with patients, doctors, nurses and GPs to improve ward-based pharmaceutical care
- Provide on-call/after-hours coverage including weekends, bank holidays and Christmas
Key requirements
- Masters Degree in Pharmacy (MPharm)
- Certificate in Pharmacy Practice or equivalent
- GPhC registration
- Post-graduate experience in clinical pharmacy
- Evidence of CPD, audits, and project work
- IT literacy (word processing, email, internet)
- Willingness to take responsibility
- Ability to work under pressure and in a team
- Desirable: Independent prescriber, post-graduate diploma in pharmacy practice
- Desirable: Staff management, acute hospital clinical pharmacy experience
- Excellent verbal and written communication
