We are seeking to recruit two General Practitioners to complete our large Clinical team across our organisation (SH Healthcare partners Limited) that comprises of 5 Surgeries. These Practices form the Chorley and South Ribble Primary Care Network. This position will be based at Adlington Medical Centre, Lostock Hall Village Surgery, Eaves Lane Surgery and Buckshaw Village Surgery.

Main duties of the job

  • Conduct face-to-face, remote, and digital consultations, using modern general practice triage systems to assess, prioritise, and manage patient care safely and effectively.
  • Provide expert clinical assessment, diagnosis, and management of patients' health needs.
  • Apply clinical judgement within total triage models (GP led clinician-led triage) to ensure patients are directed to the most appropriate service or clinician.
  • Provide clinical leadership in quality improvement initiatives.
  • Accurately record clinical assessments, recommendations, and outcomes on GP clinical systems and in personal clinical notes, in line with practice policies and information governance standards.
  • Participate in regular clinical supervision to support continuous professional development and shared learning.
  • Home visits will be required.

Supportive Clinical Duties

  • Actively contribute as part of the multidisciplinary team, attending relevant MDT and network meetings and providing clinical input and feedback.
  • Report clinical incidents and contribute to investigations, root cause analyses, serious incident reviews, and multidisciplinary case discussions.
  • Critically appraise clinical evidence and present findings to colleagues at prescribing meetings, educational sessions, and practice learning events, as required.
  • Undertake clinical audits and support or supervise audit activity conducted by other team members.

Our Primary Care Network comprises of Buckshaw Village Surgery, Eaves Lane Surgery, Adlington Medical Centre, Croston Medical Centre and The Village Surgery at Lostock Hall. Our total patient list size is combined at 24,500 and is growing rapidly. We currently employ a total of 83 members of staff; we take pride in working together to deliver the best care for our patients. We were rated Good in all areas at our last CQC inspection. The practice is constantly evolving to take advantage of the opportunities arising in the ever-changing NHS, and to find new and innovative ways of working.

Person Specification

Experience

  • Good understanding of primary care structure and General Practice.
  • Good clinical knowledge.
  • Able to apply clinical knowledge appropriately in a variety of practice settings or patient groups.
