Senior Clinical Pharmacist -Dedicated Ward Role in Chesterfield

Are you a clinical pharmacist looking to embed yourself into the multi-disciplinary team, to influence prescribing at the point of decision making, and make a real difference to patient care by optimising medicines? We are looking for ambitious and dynamic pharmacists to join our clinical pharmacy team.

We have a well-established Dedicated Ward Pharmacist (DWP) model at Chesterfield Royal, realising the significant benefits to our patients. Enhanced medicines optimisation, facilitating safe and effective patient flow and transition of care are key priorities of the DWP team. We are looking for pharmacists who will embrace these priorities and put patient-centred care at the heart of all their activity. Good communication, time management and analytical skills will be required, along with the ability to work effectively and communicate within a team. You will also contribute to the provision of weekend pharmacy services, again working at ward level.

Main duties of the job:

• Provide dedicated ward pharmacy (DWP) services to a given ward or provide DWP backfill across a range of medical and surgical specialties.
• Support ward rounds to influence clinical and cost-effective prescribing.
• Manage the process of discharge prescribing to improve patient flow and ensure optimal safe transfer of care around medicines.
• Be supported by an innovative group of medicines management technicians.
• Develop your clinical, leadership and management skills to the highest level.
• Participate in clinical supervision and mentoring, service evaluations and audits to develop your skills and aid your career progression.

Job responsibilities:

• Provide clinical pharmacy services to patients, overseeing the use of medicines in terms of safety, efficacy, efficiency and economy.
• Identify and address medicines governance issues that arise from clinical service delivery.
• Take a proactive approach to the management of ward discharges, including prescribing of discharge medicines in line with local guidance.
• Work collaboratively with medical and nursing teams and Trust Operations Team to facilitate and expedite discharge.
• Ensure high-quality patient counselling is delivered by the MDT during admission and at discharge.
• Undertake medicine optimisation activities in line with clinical specialty.
• Provide clinical pharmacy support to consultant ward rounds, MDT meetings and ward operational meetings.
• Review and investigate medicines incidents and liaise with the ward team to adopt changes in practice where necessary.
• Take a lead role in managing ward-based medicines expenditure.
• Contribute to the delivery and development of clinical pharmacy services across the Trust.
• Actively participate within the multi-disciplinary and clinical pharmacy team.
• Provide supervision, training and tuition of pre-registration students and pharmacists.
• Participate in clinical audit and other initiatives aimed at assuring high-quality prescribing and pharmacy practice.
• Provide training and education to other healthcare professionals.
• Maintain confidentiality at all times relating to patients, staff and the Trust.

Person Specification:

• Qualifications and Training: MPharm, Working towards Diploma/MSc Clinical Pharmacy, GPHC registration.
• Experience: 18 months post-registration in acute hospital Pharmacy, Evidence of relevant CPD, Experience of influencing cost and quality of prescribing.
• Skills and Knowledge: Wide-ranging clinical skills/knowledge, Teaching Skills, Communication and Time Management Skills.
• Personal Attributes: Communication and Influencing Skills, Reliable and Conscientious, Resilience.
