Overview
In this Clinical Pharmacist role, you will contribute to medicines optimisation across three practices within the PCN, delivering structured medication reviews and supporting long-term condition management. You will collaborate with GPs, nurses and allied staff to improve patient safety, reduce waste and enhance prescribing quality. The role blends patient-centred care with service development, aiming to deliver high-quality, cost-effective medicines management. Responsibilities
- Undertake structured medication reviews and medicines optimisation aligned with local and national guidance
- Handle complex medication queries from patients and professionals
- Review and optimise medicines for long-term conditions
- Provide pharmaceutical advice to clinicians and patients
- Monitor and review high-risk medications
- Reconcile medicines post-discharge from hospital and other care settings
- Identify and address medication-related risks and prescribing issues
- Promote evidence-based, cost-effective prescribing and reduce prescribing waste
- Support patients to improve adherence and understanding of treatments
- Provide medicines management support to care homes
- Implement national and local prescribing guidance
- Develop strong working relationships with community pharmacy, secondary care and community services
Key requirements
- Master of Pharmacy (MPharm) or equivalent
- Current GPhC registration
- Independent Prescriber qualification or actively pursuing
- PCPEP completion or commitment to complete
- Minimum two years post-registration pharmacy experience
- Experience in patient-facing clinical services and medicines optimisation
- Experience working in multidisciplinary teams
- Excellent clinical pharmacy knowledge and communication skills
- Knowledge of prescribing systems and clinical governance
- Strong organisational and workload management abilities
