Overview
In this role you will provide autonomous clinical care within a multidisciplinary primary care team, managing acute and chronic medical conditions, promoting health, and supporting policy review. You will work to deliver high-quality patient care, contribute to chronic disease programs, and uphold practice targets within a collaborative, innovative environment. This is a chance to shape care delivery for a large patient population in a modern, paper-light setting.

Responsibilities
- deliver effective medical care to the eligible population
- manage patients with acute and chronic conditions with expert advice
- design and review chronic disease treatment plans
- identify and support patients at risk of long-term conditions
- provide advanced nursing care within evidence-based guidance
- order and interpret pathology tests as needed
- run chronic disease clinics and refer to specialist care when needed
- maintain accurate clinical records and ensure proper read codes
- maintain chronic disease registers and prioritise health issues
- support the team during clinical emergencies
- recognise and refer mental health needs
- act as extended/supplementary prescriber within guidelines
- support medication use reviews and annual reviews
- contribute to practice targets (QOF) and local guidance
- accept delegated QOF responsibilities
- attend practice meetings and liaise with external services
- safeguard and substance misuse policies adherence
- prepare revalidation and commit to CPD
- deliver opportunistic health promotion
Key requirements
- Master level education per HEE framework for advanced clinical practice
- Qualified independent prescriber - V300
- Registered health care professional with UK registration
- Experience in primary care environment
