As an Enhanced Clinical Practitioner (ECP), you will be part of our valued team in the Hospital at Home service based across community bases within Swindon or Wiltshire. Your main responsibilities will include delivering high-quality, evidence-based enhanced clinical care using advanced assessment skills in a community-based environment. Your role is crucial in preventing unnecessary hospital admissions and assisting with early, safe discharge from hospitals, working within a multidisciplinary team.

Essential requirements include:

  • A current professional registration with the NMC or HCPC
  • A relevant degree with postgraduate learning or equivalent experience
  • Significant experience in elderly care, frailty, community services, or acute specialties
  • Strong clinical judgement and excellent communication skills
  • The ability to work autonomously and leadership experience
  • A full driving license and a car
  • Experience with electronic clinical systems is desirable

Main Responsibilities:

  • Deliver high-quality, evidence-based enhanced clinical care
  • Maintain oversight of patient care, identifying and responding to deterioration
  • Provide clinical leadership within the multidisciplinary team
  • Contribute to supervision, education, and workforce development
  • Participate in audit, quality improvement, and service development

The ideal candidate will have:

  • Current professional registration with the NMC or HCPC
  • Degree-educated with relevant postgraduate learning or equivalent experience
  • Recognised qualification in enhanced/advanced clinical assessment and diagnostic reasoning
  • Significant post-registration experience in elderly care, frailty, community services, or a relevant acute specialty
  • Proven ability to independently manage complex caseloads
  • Excellent communication skills and strong clinical judgement
  • Confident IT skills and experience using electronic clinical systems
  • Commitment to safe practice, professional accountability, and clinical governance
