Overview

In this role you will join the Frailty Virtual Ward to lead comprehensive geriatric assessments and develop personalised care plans within the MDT. You will work in the community to prevent hospital admissions and support patients at home, collaborating with UCRT and district nurses. The post offers opportunities for learning and professional growth with consultants, and a focus on improving patient flow and discharge planning. You will inspire teams, drive new approaches, and ensure high-quality clinical care in a collaborative, dynamic setting.

Responsibilities

  • Undertake comprehensive geriatric assessments and formulate personalised management plans with the MDT
  • Lead on staff development and supervise junior colleagues; support discharges to optimise length of stay
  • Attend virtual GP calls and liaise with Frailty Consultant for complex medical management
  • Develop innovative ways to increase patient flow and support admission avoidance with UCRT
  • Agree outcomes for people with frailty and complex co-morbidities to reduce admissions
  • Collaborate with MDT and services across community settings to deliver high-quality care
  • Provide leadership to maintain high standards of clinical care and support training
  • Participate in multi-disciplinary team meetings and continuous professional development

Key requirements

  • Current registration with relevant professional body
  • Qualified Independent Prescriber / willingness to attend training
  • Management education development including supervision/teaching/team management
  • Desirable: ACP accredited course
  • At least 3 years of community experience
  • Minimum 5 years post-registration experience
  • Commitment to working as part of a multi-disciplinary team
  • Full driving licence and access to a car
  • Excellent interpersonal skills
