Overview
In this Advanced Clinical Practitioner role at Newquay Community Hospital, you will deliver autonomous, patient-centred care for older adults with frailty and complex needs.
You’ll lead advanced assessment, diagnosis, treatment and prescribing within a multidisciplinary team, aiming to improve patient flow and safe discharge.
You will shape care pathways, drive service improvements, and mentor colleagues while collaborating with health and social care partners to deliver care closer to home.

Responsibilities
- perform advanced assessment, diagnosis and management of acute, chronic and complex conditions
- manage an autonomous caseload including admission assessments, patient reviews and independent prescribing
- coordinate care with medical, nursing, therapy, primary care, social care and community teams to support safe discharge
- develop and implement care pathways for older people and multimorbidity
- lead quality improvement initiatives and contribute to governance, audit and evidence-based practice
- undertake advanced clinical procedures and monitor progress, initiating investigations as needed
- provide clinical leadership, supervision and role-modelling to colleagues
- support education, mentorship and professional development of staff
- Key requirements
- RN Adult (current registration)
- Independent Non-Medical Prescriber (NMC)
- Masters in Advanced Nurse Practitioner or equivalent portfolio
- UK professional registration (NMC)
- recognised teaching/mentorship qualification (level 3)
- experience in gerontology or senior-level relevant medical specialty
- ability to interpret ECG and X-ray
- disclosure and barring service check (enhanced)
