Overview

In this rotational role, you support medicine optimisation across a broad portfolio of specialties, helping to identify and resolve medication-related issues and ensure harm-free care.

You will work within an MDT, advising on safe, cost-effective prescribing and supporting discharge information.

The role offers development toward a Diploma in Clinical Pharmacy or non-medical prescribing, with progression from Band 6 to 7.

It suits newly qualified pharmacists or those changing sector seeking diverse clinical experience and mentorship.

Responsibilities

  • Medicine optimisation for admitted patients across multiple specialties
  • Identify medication-related issues contributing to admission and reduce prescribing risks
  • Conduct ongoing prescribing reviews to maintain harm-free care
  • Provide expert medication management advice to medical staff
  • Support discharge planning with primary and community care providers
  • Collaborate within a multidisciplinary team including specialist nurses and consultants
  • Demonstrate efficient, autonomous work with strong communication and organisational skills
  • Adapt to a seven-day rota with flexible working hours
  • Key requirements
  • Vocational Masters degree in Pharmacy or equivalent
  • Registered with the General Pharmaceutical Council of Great Britain
  • Completed foundation pharmacist training and passed registration exam (desirable)
  • NHS hospital pharmacy knowledge
  • Ability to work both independently and as part of a team
  • Commitment to continuing professional development / education
  • Computer literate
  • Excellent verbal and written communication skills
  • Ability to analyse drug charts and patient information for medication advice
  • Good time management and influencing skills
  • Sufficient hospital pharmacy experience (desirable)
