Our CYPS Mindworks Hub East is a highly supportive, friendly and innovative team covering the East of Surrey (Leatherhead and Redhill). As part of our i-THRVE framework, this team is focused on Getting More Help and Risk Support to Children and Young People. You would join a strong multidisciplinary team offering assessments and interventions to children and young people experiencing psychological distress, emotional and mental health difficulties as well as providing support to our Partner Agencies. We are seeking those professionally registered with NMC, HCPC or SWE and have significant experience of working with under 18-year-olds.
Main duties of the job:
- Provide timely interventions that contribute to the care of young people with mental health problems and their families.
- Put the experience of the CYP using the service at the centre of decision making whilst building a supportive and responsive relationship with colleagues.
- Conduct Mental Health Assessments and Risk Assessments, implementing risk management plans.
- Deliver evidenced based Group interventions.
- Use outcome measures to evidence good practice.
- Supervise and support student nurses.

Job responsibilities:
- Work closely with the Mindworks Alliance including School Based Needs, Surrey Well Being Partnership and other agencies during office hours, responding to referrals of young people presenting in need.
- Work as an autonomous practitioner, undertaking clinical assessment of children and young people who may have a complex co-morbid mental health need.
- Be an integral part of the Hub duty Rota, responding to queries from families and the wider network including risk support.
- Manage own caseload from referral to discharge/transfer, holding medium to high-risk cases in liaison with the MDT.
- Ensure that outcome measures are implemented and audited.
- Contribute to relevant research and ensure that interventions on the pathway are evidence based.
- Be responsible for the oversight and completion of initial assessment and subsequent facilitation of a warm handover and referrals to other services and/or discharge with a signed care plan regarding safety.
Person Specification:
- Qualifications: Registered professional with current registration, appropriate to the job role (NMC, HCPC or SWE). Relevant professional qualification and registration in Nursing, Social Work or Occupational Therapy.
- Experience: 2-year post qualification experience of working in the community with young people with mental health difficulties and their families. Experience and ability to undertake Mental Health Assessments, Risk Assessments and use outcome measures to evidence good practice.
