Advanced Clinical Practitioner - Urgent Care in Brighton

Do you want to work within an innovative, forward-thinking and progressive integrated urgent care team? gtd healthcare is one of the largest, not-for-profit, NHS commissioned primary and urgent care providers in North West England. We have an excellent opportunity for two Advanced Clinical Practitioners (ACP's) to join our urgent care teams at Royal Preston and Chorley Hospitals on a full-time basis; part-time hours will also be considered.

At Royal Preston and Chorley Hospitals, we provide varied service lines, which include DVT clinics, minor injuries, face-to-face walk-in patients, NHS 111 appointments, remote clinical assessment calls and the triage function between the emergency departments and urgent care centres. Our team is made up of ACP's with a blend of illness and/or injury training from a variety of backgrounds, such as nurses and paramedics. You will work alongside established ACP's, Senior ACP's, GP's or middle-grade doctors, as well as having a supportive administrative team who help coordinate care and flow of patients. Main duties of the job:

• Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
• Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
• Provide information and advice on prescribed or over-the-counter medication and medication regimes, side-effects and interactions.
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Refer and discharge safely ensuring that safety-netting advice is offered.
• Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Person Specification:

Qualifications:

• Essential: Registered healthcare professional, for example; first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or equivalent.
• V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility (or currently undertaking).
• Desirable: Mentor/Teaching qualification. Clinical supervision training and experience.

Experience of leading/managing quality improvement. Experience of use of Digital technology and video consultations. Experience of effective time management during consultations. Experience of consulting with adults and children.
