Overview
In this role you will provide clinical leadership and line management for the Virtual Services Team within the Virtual Ward, aiming to deliver care at home, improve quality of life, and reduce hospital stays. You will lead cross-organisational collaboration with social services and primary care, support safe community-based care, and drive staff development. You will be a visible, authoritative mentor who influences discharge planning and care pathways, while aligning with Trust values and NHS targets. This is a chance to shape virtual care delivery at scale.

Responsibilities
- Coordinate and manage the Virtual Services team and foster strong inter-agency relationships
- Provide clinical leadership and line management to staff
- Maintain a professional standard and develop staff to the highest levels
- Deliver high-quality communication to staff, patients, and families
- Create and sustain a learning environment meeting staff development needs
- Promote safe, effective use of virtual care pathways across the Trust
- Collaborate across organisations, including social services and primary care, with families and carers
- Lead by example, motivate the team, and ensure accessible support for discharge and care needs
Key requirements
- Significant post-registration experience in an acute trust or community setting
- Experience in MDT working and across organisational boundaries
- Experience of leading and managing change
- Experience in clinical audit and using outcome measures
- Experience in policy development
- RN (Adult) with current NMC registration
- Supporting Learning in Clinical Practice (SLIP) or equivalent
- UK driving license
- Degree or equivalent level of education or experience
- Mentoring and advanced communication
- Leadership and team collaboration
- Time management and organisation
- Report writing / presentation skills
- Computer literacy: Word/Excel/PowerPoint
- Clinical credibility
