Overview
In this Care Co-ordinator role you will be a welcoming first point of contact, helping patients access appropriate services and ensuring smooth daily clinic operations. You work across patient-facing and administrative tasks, handling enquiries, appointments, and records with care and confidentiality. You’ll thrive in a multidisciplinary team, staying calm under pressure and delivering excellent patient service. This role supports the Practice’s values and enables high-quality care for the local community.

Responsibilities
- Provide a friendly, efficient care navigation service to patients and visitors
- Handle general enquiries and assist with appointments and Practice services
- Manage urgent cases promptly and appropriately
- Support new patients with registration procedures and requirements
- Process home visit requests and refer to the appropriate clinician
- Receive and make telephone calls with accurate message handling
- Update patient information and electronic records in line with data protection
- Process prescriptions per Practice procedures and maintain records
- Advise on charges for private services and complete related documentation
- Assist with opening/closing the Practice and security tasks
Key requirements
- GCSE English and Maths grade C/4 or above
- Experience of working with the general public
- Experience with administrative duties
- Excellent written and verbal communication skills
- Strong general IT skills
- Effective time management and organisational skills
- Ability to work in a team and independently
- Good interpersonal and problem-solving skills
- Commitment to following policies and procedures
- Sensitivity and empathy when dealing with distressed patients
- Flexible to work across two sites and outside core hours
- Willingness to undertake DBS check
- Confidentiality commitment
- Experience in healthcare or general practice and knowledge of SystmOne is advantageous
